Subject: [PATCH] Update ISSUE_TEMPLATE.md (#3953)

Adds fenced code blocks / output blocks to the `Steps to reproduce`, `Expected behavior` and `Actual behavior` sections, so as to facilitate proper formatting.

Fix #3952.
