Fix sgx_create_enclave retry mechanism when load_enclave
just return SGX_ERROR_MEMORY_MAP_FAILURE. Deprecated the
SGX_ERROR_MEMORY_MAP_CONFLICT. Make the retry mechanism
compatible with new driver and urts.

The templated "storage" field in randomly_placed_buffer is uses alignas
in an undefined way. The field is also not used, so this change removes
it.

The sgx_uswitchless.h header is used in both trusted and untrusted
contexts from transitive inclusions. The sgx_urts.h header is strictly
an untrusted domain header. Users that depend on the untrusted runtime
should not depend on the transitive include behavior to provide
sgx_urts.h.
