Fixed irep index overflows, when defining new closure.

Issue #1137 .
In the present implementation, when calling mrb_load_string continuously, irep is not released.
Therefore, a 16-bit index was overflowing.

Remove str_to_mrb_int(). There is some reasons.
...
It is not used in the core.
It does not have mrb_ prefix.
strtol() is slightly heavy and we have similar API.
