Implement Karatsuba algorithm for operands with 32-99 limbs, providing
~10-25% speedup over schoolbook multiplication in this range.

Algorithm hierarchy is now:
  - Schoolbook: < 32 limbs
  - Karatsuba:  32-99 limbs
  - Toom-3:     >= 100 limbs
