khash: move kh_alloc to internal helper

Move kh_alloc_##name from public API to internal helper kh__alloc_##name
since it's only used internally within khash implementation.

Changes:
- Remove kh_alloc_##name from KHASH_DECLARE
- Add kh__alloc_##name as static inline in KHASH_DEFINE
- Update internal calls to use kh__alloc_##name
