Fixed constant (and class variable) reference bug; fix #3839

Unlike method definition, constant reference should start from
`MRB_PROC_TARGET_CLASS(ci->proc)`, not `ci->target_class`.
In addition, `MRB_PROC_TARGET_CLASS(ci->proc)` is always set.
